What We Do:
Supportive Services for Veteran Families-SSVF
On October 1st, 2013, the SSVF Grant will expand to serve Clay and Nassau Counties. This expansion will also welcome Community Connections and Barnabus Center to our team of sub-grantees.
SSVF provides case management and temporary financial assistance to individual Veterans or Veteran families experiencing homelessness or extreme housing crisis.
- Case Management: Case management can be the most valuable resource for someone who is experiencing homelessness, or who is on the brink of homelessness. Case Managers help to identify barriers to stable housing, provide appropriate referrals to cover needs (such as job placement assistance, child care, SNAP, etc), and help clients develop a plan to remain stably housed. All eligible SSVF clients will be assigned a case manager.
- Prevention Assistance: This funding is set aside specifically for Veterans who will become literally homeless without SSVF. Households must meet strict minimum eligibility to qualify for prevention funds.
- Rapid Re-housing: Rapid Re-housing provides financial assistance to help literally homeless veterans move from the streets or emergency shelter into housing as quickly as possible. Households must meet strict minimum eligibility to qualify for rapid re-housing funds.
If you, or someone in your family is a Veteran, please call 904-354-1100 ex3 to speak with our Screening Specialist.
